1. How many loans or credit cards have you applied for in the last 6 months?
(Loans include: personal bank loans, student loans, auto loans and mortgages)
2. When did you last miss a loan or credit payment (i.e., you received a negative item on your credit report)?
3. Provide the percentage of total outstanding balances currently being carried on your open credit cards to the total amount of credit limits on those cards.

For example, if you have two credit cards:

  • One with balance of $650 and limit of $2,000

  • One with balance of $830 and limit of $4,000

  • Your total credit card balances would be $1,480 ($650 + $830), your total credit limits would be $6,000 ($2,000 + $4,000), and the percent of your total credit limits that your credit card balances represent are: $1,480 divided by $6,000, or 25%.
    4. How many types of credit do you have (open or closed) by stating the amount in each line. If zero, leave blank.
    (e.g., if you have 3 credit cards, put the number 3 in the credit card field).
